Sample Procedure Statement for Reporting of Violations of Drug and Alcohol Policy

If an employee of ABC has been convicted under a criminal drug statute for violations occurring on or off company premises while conducting ABC business, a report of the conviction must be made within five (5) days after the conviction. Report of such convictions must be made in writing.
