Documenting Accommodation Requests and Compliance

The handling of requests for accommodation and the assessment of the costs and benefits of various options should be fully documented. Adequate documentation would include such information as: - When and how a request for accommodation was made; - The specific need of the person with a disability; - Possible accommodations suggested by the employee; - Additional possible accommodations suggested by the employer; - An analysis of the costs and benefits of each possible accommodation; - The reason a specific accommodation was chosen; - The reason other accommodations were rejected; and - The reason all optional accommodations were rejected.
